This policy explains what happens to information you send us through this website. It covers the website only. Information you give us as a patient inside the practice is handled under HIPAA and under the practice's Notice of Privacy Practices, which is a separate document available at the front desk.
What we collect
If you fill in the appointment request form, we receive the name, phone number, preferred language, preferred day or time, and the short description you type into the "What can we help with?" field. We use these only to contact you and arrange an appointment.
We ask you not to send symptoms, diagnoses, medication lists, insurance ID numbers, or other detailed health information through this form. It is an ordinary web form, not a secure patient portal.
What we do with it
- We contact you to schedule or confirm an appointment.
- We do not sell your information.
- We do not share it with advertisers or data brokers.
- We share it with service providers only where needed to operate the site or return your call, and only to that extent.
Cookies and analytics
This site sets no cookies of its own, runs no analytics, and carries no advertising or remarketing pixels. Nothing here tracks you across other websites.
Two things on the page are loaded from Google: the typefaces, and the map on the "Visit Us" section. The map is an embedded Google Maps frame, and Google may set its own cookies when it loads. That is Google's processing under Google's privacy policy, not ours, and we receive nothing from it.
If analytics or call-tracking is ever added to this site, this section will be updated to say so before it goes live.
Your choices
You can ask us to delete an appointment request you sent through this site. Call the office at (313) 398-4269 and we will remove it.
